Effects of sacubitril valsartan sodium combined with dapagliflozin on cardiac function and left ventricular remodeling in patients with acute myocardial infarction and type 2 diabetes mellitus

Abstract：Objective To explore the effect of sacubitril valsartan sodium combined with dapagliflozin on cardiac function and left ventricular remodeling in invalids with acute myocardial infaraction(AMI)and type 2 diabetes mellitus(T2DM).Methods A total of 120 patients with AMI and T2DM treated in Cangzhou People's Hospital from June 2021 to December 2023 were ran-domly divided into two groups by random number table method.After routine treatment was performed,the patients in the control group were given sacubitril valsartan sodium and those in the observation group were given dagliprazin.The treatment course was 6 months in both groups.The cardiac function and left ventricular remodeling indexes,6-min walking test,and the occurrence of major adverse cardiovascular events and drug-related adverse reactions were compared between the two groups before and after treatment.Results After treatment,the LVEF was(52.15±5.02)%and(48.62±4.78)%in the observation group and control group,which was higher than that before treatment((44.32±4.57)%and(45.27±4.13)%).Meanwhile,it was higher in the observation group than in the control(t=3.945,P<0.05).The LVMI,LVEDD and LVESD indicators of the observation and the control group((102.21±11.73)g·m-2 vs(108.65±12.18)g·m-2,(51.32±4.19)mm vs(42.45±3.46)mm,(54.41±4.82)mm vs(45.17±3.25)mm)after treatment were all lower than those before treatment((122.47±13.14)g·m-2 vs(121.15±13.53)g·m-2,(58.48±4.87)mm vs(48.49±3.84)mm,(57.29±4.56)mm vs(49.12±3.14)mm).These indicators were lower in the observation group than in the con-trol(t=2.950,3.748,4.438,P<0.05).The 6-min walking test levels in the observation group and control group after treatment((452.64±33.75)m vs(417.23±31.87)m)were higher than those before treatment((243.51±56.87)m vs(239.75±55.42)m)and they were higher in the observation group than in the control(t=5.909,P<0.05).The total incidence of major adverse cardiovascular events was 8.33%in the observation group,which was lower than that of 23.33%in the control(χ2=5.065,P<0.05).The total inci-dence of drug-related adverse reactions was 10.00%and 13.33%in the observation group and control group,showing no significant difference between the two groups(χ2=0.323,P=0.570).Conclusion The combination of sacubitril valsartan sodium and dapagli-flozin in treating patients with AMI and T2DM can significantly enhance cardiac function,ameliorate left ventricular remodeling,decrease the occurrence of major adverse cardiovascular events and exhibit a favorable safety profile.
