Efficacy and safety of palbociclib combined with aromatase inhibitors in advanced HR+/HER2-postmenopausal breast cancer

Abstract:Objective To investigate the safety and effectiveness of aromatase inhibitors in combination with palbociclib in postmenopausal women with advanced breast cancer of the hormone-receptor-positive/human epidermal growth factor recep-tor-2-negative (HR+/HER2-) subtype. Methods A total of 110 postmenopausal patients with advanced HR+/HER2-breast cancer admitted to Shangqiu First People's Hospital from February 2019 to June 2024 were selected as the study objects. Using the random number table method,these patients were divided into the experimental group and control group,with 55 in each group. The patients in the control group were treated with aromatase inhibitor letrozole and those in the experimental group with Palbociclib in addition to letrozole for 7 treatment cycles. The patients' quality of life and pain level were assessed,the expression levels of serum tumor markers carbohydrate antinegen 153 (CA153) and carcinoembryonic antigen (CEA) were detected before and after treatment,and the drug efficacy and adverse reactions of the two groups were evaluated. Results After treatment,the experimental group possessed a higher objective response rate (ORR) as compared with the control (94.55%(52/55) vs 56.36%(31/55),x2=47.632,P<0.05). Tthe scores of physiological function,mental health,vitality,general health and physiological function of the two groups were higher after than before treatment,and the scores of physiological function,mental health,vitality,general health and physiological function of the experimental group were higher than those of the control (t=13.048,15.386,15.523,8.152 and 7.045,all P<0.05). The Visual an-alogue scale(VAS) score of the two groups was significantly decreased after treatment,with a lower score in the experimental group as compared with the control ((6.02±0.95) scores vs (6.84±1.07) scores) (t=-4.250,P<0.05). The expression levels of serum CA153 and CEA in both groups were significantly decreased after treatment,with lower expression levels in the experimental group than in the control ((13.20±1.58) U·mL-1,(19.54±6.04) μg·L-1 vs (18.54±1.61) U·mL-1,(23.14±5.47) μg·L-1) (t=-17.556,-3.276,P<0.05). Adverse reactions such as anemia,reduced neutrophils,fatigue and abnormal liver and kidney function were observed in both groups. Compared to the experimental group,the control group experienced a substantially reduced frequency of adverse responses (98.18% (54/55) vs 41.82% (23/55),x2=41.602,P<0.001). The progression-free survival was significantly shorter in the control group than in the experimental group ((3.54±1.33) months vs (6.84±1.56) months,t=11.938,P<0.001). No significant difference of stable case incidence was found between two groups (18.18% (10/55) vs 25.45% (14/55),x2=0.853,P=0.356). There were less relapsed cases in the experimental group than in the control (12.73% (7/55) vs 34.54% (19/55),x2=7.253,P=0.007). Conclusion Compared with aromatase inhibitors alone,palbociclib combined with aromatase inhibitors has a better effect on patients with HR+/HER2-post-menopausal advanced breast cancer,with significantly improved quality of life and pain,reduced tumor marker levels,and a better prognosis. Although this combination therapy increases the incidence of neutropenia,this adverse effect is reversible.
