Application value of remifentanil combined with sufentanil in patients undergoing percutaneous nephrolithotomy

Abstract:Objective To explore the application value of remifentanil combined with sufentanil general anesthesia in pa-tients undergoing percutaneous nephrolithotomy (PCNL). Methods A total of 100 patients undergoing PCNL in Suzhou Hospital affiliated to Nanjing University School of Medicine from January 2022 to March 2024 were selected as the research subjects and divided into two groups using a random number table method,with 50 in each group. According to the dropout criteria,the study was completed in 37 patients in the control group and 45 in the experimental group. The patients in the experimental group were given remifentanil combined with sufentanil anesthesia while those in the control were given remifentanil anesthesia. The anes-thesia effect and postoperative adverse events as well as the changes in hemodynamics and stress indicators before anesthesia (T0),before tracheal intubation (T1),after tracheal intubation (T2),and at the end of surgery (T3) were compared between the two groups. Results The time for spontaneous breathing recovery,consciousness clarity,and tracheal extubation in the experimental group were (3.85±0.84) min,(6.81±1.03) min,(4.21±0.52) min,which were lower than those of (5.76±0.59) min,(9.46±1.50) min,(6.04±0.51) min in the control,showing statistically significant differences (t=11.661,9.452,15.996,all P<0.05). Repeated measures analysis of variance showed that heart rate (HR),mean arterial pressure (MAP),noradrenaline (NE) and epinephrine (E) had statis-tically significant differences in time,group,and the interaction between time and group (P<0.05). The incidence of adverse events was significantly lower in the experimental group than in the control (4.44% vs 27.03%,x2=8.289,P<0.05). Conclusion The com-bination of remifentanil and sufentanil can effectively reduce intraoperative hemodynamic fluctuations and stress mediator secretion in PCNL patients,resulting in shorter postoperative recovery time and lower incidence of adverse events.
