Efficacy of intravenous administration of human immunoglobulin as an adjuvant treatment for late-onset sepsis in neonates and its effect on inflammatory factors

Abstract:Objective To investigate the efficacy of intravenous administration of human immunoglobulin (IVIG) as an ad-juvant treatment for late-onset sepsis in neonates and its effect on inflammatory factors. Methods A total of 110 neonates with late-onset sepsis admitted to Hai'an People's Hospital from February 2021 to February 2024 were enrolled in this study and divid-ed into the observation group and control group using a random number table method,with 55 in each group. The patients in the control group were given antibacterial drugs while those in the observation group were given intravenous administation of human immunoglobulin based on the regimen in the control. All patients in both groups were treated continuously for 5 days. The clinical efficacy,levels of inflammatory factors and immune function indicators before and after treatment,improvement time of symptoms and signs,hospitalization time,and incidence of adverse reactions were compared between the two groups. Results Compared to the control group,the observation group's clinical effective rate was greater (87.27% (48/55) vs 65.45% (36/55),x2=7.253,P<0.05). After treatment,the serum levels of amyloid A (SAA),procalcitonin (PCT),and C-reactive protein (CRP) in the two groups were all reduced. The levels of SAA,PCT and CRP in the observation group were (17.91±6.47) mg·L-1,(3.18±0.56) ng·mL-1 and (8.25±1.36) mg·L-1,which were lower than those of (22.50±6.52) mg·L-1,(4.73±0.71) ng·mL-1 and (10.88±2.05) mg·L-1 in the control (t=3.706,12.712,7.928,all P<0.05). The levels of IgG and IgA in the two groups were increased but that of IgM decreased after treatment. The levels of IgG and IgA in the observation group ((13.51±4.03) g·L-1,(0.33±0.11) g·L-1) were higher than those of the control group ((10.86±3.81) g·L-1,(0.22±0.09) g·L-1),and the IgM level of the observation group ((0.20±0.07) g·L-1) was lower than that of the control group ((0.27±0.11) g·L-1),and the differences were statistically significant (t=3.544,5.740,3.982,all P<0.05). The improvement time of milk rejection,resuming normal feeding,body temperature returns to normal and hospital stay time in the observation group ((5.14±1.75) d,(3.93±1.46) d,(8.76±2.47) d,respectively) were remarkably shorter than those in the control group ((7.50±2.36) d,(6.21±2.30) d,(11.24±3.79) d,respectively) (t=5.957,6.207,4.066,all P<0.05). There was no statistically sig-nificant difference in the incidence of adverse reactions between the two groups (10.91%(6/55) vs 12.73%(7/55),x2=0.768,P>0.05). Conclusion IVIG is effective in adjuvant treatment of late-onset sepsis in neonates since it can reduce the levels of inflammatory factors and improve the body's immune function and symptoms.
