Efficacy and safety of minocycline combined with methylprednisolone pulse therapy for refractory mycoplasma pneumoniae pneumonia in children

Abstract:Objective To evaluate the safety and effectiveness of methylprednisolone pulse therapy in conjunction with mino-cycline for the treatment of children's refractory mycoplasma pneumoniae pneumonia (RMPP). Methods One hundred and six-teen children with RMPP admitted to Nantong First People's Hospital from January 2023 to August 2024 were selected and divided into a control group and a combined group according to the random number table method,with 58 in each group. Children in both groups routinely received supportive treatments. The children in the control group were given intravenous infusion of azithromycin. On this basis,the children in the combined group were given oral minocycline and methylprednisolone sodium succinate pulse ther-apy. The clinical efficacy,symptom disappearance time,the occurrence of adverse reactions during treatment,serum tumor necrosis factor-α (TNF-α),C-reactive protein (CRP),T cell immunoglobulin mucin molecule 3 (TIM-3) levels before and after treatment were compared between the two groups. Results The total effective rate was 87.93%(51/58) in the combined group,which was higher than that of 70.69%(41/58) in the control (x2=5.254,P<0.05). The time for symptoms such as fever,cough,and lung rales to disap-pear was (6.83±1.25) d,(6.59±1.24) d and (6.14±2.13) d in the combined group and it was significantly shorter than that of (11.34±1.97) d,(9.46±1.57) d and (9.78±2.65) d in the control (t=14.722,10.925,8.154,all P<0.05). After treatment,the serum TNF-α,CRP and TIM-3 levels in the combined group were (45.26±6.31) pg·mL-1,(13.64±4.57) mg·L-1,and (5.74±1.13) ng·L-1,which were lower than those of (57.61±6.02) pg·mL-1,(26.76±6.94) mg·L-1,(8.69±1.21) ng·L-1 in the control (t=10.785,12.025,13.570,all P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (6.90%(4/58) vs 15.52%(9/58),x2=2.166,P>0.05). Conclusion Minocycline combined with methylprednisolone pulse therapy has good clinical efficacy in the treatment of pediatric RMPP since it can significantly shorten the course of the disease in children,reduce the serum TNF-α,CRP,and TIM-3 levels in children. Meanwhile,it is of high safety.
