Mediating effect of hope level and coping style on disease uncertainty and medication compliance in patients with COPD

Abstract：Objective:To investigate the current status and related factors of drug treatment compliance in patients with chronic obstructive pulmonary disease(COPD)and analyze the mediating effect of hope level and coping style on disease uncertainty and drug treatment compliance in them.Methods:A total of 108 patients with COPD who were admitted to our hospital from October 2022 to De-cember 2022 were selected as the study objects,and the investigation was carried out using Herth Hope Scale(HHI),Simple Coping Style Questionnaire(SCSQ),Chinese version of Mishel Disease Uncertainty Scale(MUIS)and Morisky Questionnaire(MMAS-8).The influ-encing factors of drug treatment compliance in COPD patients were analyzed by univariate and multifactorial analysis.AMOS 20.0 soft-ware was used to build a structural equation model to analyze the mediating effects of hope level and coping style on disease uncertainty and drug treatment compliance.Results:A total of 108 questionnaires were distributed in this study and the effective recovery rate was 95.37%(103/108).The HHI score of 103 COPD patients was(28.28±3.10).The positive coping score was(19.13±2.24)and the nega-tive coping score was(15.56±1.48).MUSI score was(75.75±6.46)and the MMAS-8 score was(5.78±0.42).Univariate and multivar-iate analysis showed that educational background,age,active coping,hope level,disease uncertainty and family monthly income were the influencing factors of COPD treatment compliance(t=24.492,19.632,16.821,32.677,-24.810,27.083,P<0.05).Hierarchical regression analysis showed that positive response and hope level had partial mediating effect between patients'disease uncertainty and drug treatment compliance(P<0.05).Moreover,multiple regression analysis found that hope level×active coping had significant regulating effect on patients'disease uncertainty and drug treatment compliance(P<0.05).,and the total indirect effect was 0.156,accounting for 54.30%of the total effect.Conclusion:The drug treatment compliance of COPD patients is of a low level affected by multiple factors,and there is a chain mediating effect between the level of hope and coping style on the patients'disease uncertainty and drug treatment compliance.
