Effect of ciprofol and propofol on postoperative cognitive function in elderly patients undergoing laparoscopic cholecystectomy

Abstract:Objective:To investigate the effect of ciprofol versus propofol on postoperative cognitive function in elderly patients undergoing laparoscopic cholecystectomy.Methods:A total of 150 elderly patients recieving laparoscopic cholecystectomy in our hospital from September 2022 to August 2023 were enrolled and randomized into 2 groups.Anaesthesia was induced with intravenous infusion of propofol(1.5 mg·kg-1)in the control group and ciprofol(0.4 mg·kg-1)in the observation group,with the addition of sufentanil(0.4 μg/kg)and cisatracurium(0.2 mg·kg-1)in both groups.Anesthesia was maintained using intravenous infusion of remifentanil(8~15 μg·kg-1·h-1)in both.The electroencephalographic bispectral index(BIS)values was kept between 40 and 50 by intravenous infusion of propofol(4.0~12.0 mg·kg-1·h-1)in the control group and ciprofol(0.8~2.4 mg·kg-1·h-1)in the observation one.Then the respiratory-circulatory function,cognitive function,serum level of S100 calcium binding protein β(S100β)and occurrence of adverse reactions were compared between two groups.Results:At 30 min after anaesthesia,the observation group reported significantly lower HR((68.03±8.47)times·min-1)and higher MAP((91.05±10.28)mmHg)as compared to the control group((74.82±8.35)times·min-1,(83.52±9.74)mmHg)(P<0.05).The observation group scored significantly lower in Mini-Mental State Examination(MMSE)as compared to the control one on the postoperative day 2(P<0.05).An elevation in serum levels of S100β at T1,T2 and T3 was observed in all patients compared to T0,and the elevation was milder in the observation group than in the control one(P<0.05).The observation group reported a lower incidence rate of apnea and injection pain as compared to the control one(P<0.05),while no statistical difference was found in incidence of intraoperative hypotension between the 2 groups(P>0.05).Conclusion:Ciprofol has comparable anaesthetic efficacy to propofol.However,ciprofol exhibits a more stable intraoperative circulation,can effectively reduce the impairment of postoperative cognitive function and has a positive effect on postoperative physical recovery.Therefore,ciprofol is worthy of clinical promotion and application.
