Impact of augmented renal clearance on plasma concentration of vancomycin in critically ill patients

Abstract:Objective:To observe the effect of augmented renal clearance(ARC)on plasma concentration of vancomycin in the critically ill patients.Methods:The patients who were treated with vancomycin and monitored for plasma concentration in a three-A hospital from August 2018 to April 2022 were collected,and the enrolled patients were divided into the ARC group and the normal group according to the clearance rate of creatinine.The baseline data,utilization rate of vancomycin,plasma concentration and curative effect were compared between the two groups.The related influencing factors for the plasma concentrations of the ARC group were analyzed.Results:A total of 64 patients were enrolled in the study.There were significant differences in age,blood creatinine,daily dose of vancomycin,trough concentration and control rate of trough concentration between the two groups(P<0.05).There was no significant difference in the curative effect(P>0.05).The rate achieving target trough concentration of the hyperfunction group was only 31.25%,the trough concentration was associated with single dose(r =-0.57)and dosing frequency(r = 0.581,P<0.05).Conclusion:The incidence of ARC is relatively high among the critically ill patients,vancomycin therapy often results in suboptimal blood drug concentrations in these patients,especially in younger individuals with low creatinine level.The reduction of single dose and increase of dosing frequency may raise the plasma concentration of vancomycin in the ARC patients.
