One case of dyspnea induced by ticagrelor

Abstract:One 60-year-old male patient diagnosed with acute coronary syndrome (ACS) was hospitalized for percutaneous coronary intervention (PCI) therapy. Clopidogrel (75 mg, qd) was given to the patient and the platelet aggregation rate induced by adenosine diphosphate (ADP) was 80.4%. Considered that his CYP2C19 genotype was intermediate metabolic type, clopidogrel was switched to ticagrelor (90 mg, bid). One day later, the patient developed dyspnea. Cardiac and pulmonary functions tests showed no obvious abnormality. Dyspnea induced by ticagrelor was considered by clinical pharmacist. But the patient insisted to take ticagrelor for good curative effect. Five days later, lab test showed that the platelet aggregation rate induced by ADP was 59.6%. About 14 days later, the patient complained of intolerant dyspnea. So ticagrelor was switched to clopidogrel (150 mg, qd) and dyspnea disappeared after 3 days.
