One case of anemia induced by piperacillin/tazobactam

Abstract:One 47-year-old female patient was hospitalized due to progressive enlargement of the right renal cyst for 5 years and was diagnosed as renal pelvis ureteral junction obstruction hydronephrosis, renal cyst. The patient had been healthy and had no history of drug and food allergies. The blood routine and liver function were normal at admission. Right side retrograde pyelography was performed after admission. Three days later, piperacillin/tazobactam (4.5 g, q 8 h, ivgtt) was given to her for urinary tract infection. After 5 days, results of blood routine test were as follows: hemoglobin 88 g·L-1, red blood cell count 2.85 × 1012·L-1, erythrocrit 0.26, which met the diagnostic criteria of anemia. But the patient had no obvious discomfort. The adverse reaction of piperacillin/tazobactam was considered after excluding other reasons such as infection, operation and so on. Piperacillin/tazobactam was immediately discontinued and changed to cefoperazone/sulbactam (2.0 g, q 12 h, ivgtt). Five days later, hemoglobin, red blood cell count and erythrocrit increased to 96 g·L-1, 3.21×1012·L-1and 0.29 respectively.
