Analysis of clinical pharmacists' participation in anti-infective treatment of a patient with acute necrotizing pancreatitis

Abstract:One 31-year-old female patient was admitted to hospital because of abdominal pain and abdominal distension for three days. The patient was diagnosed as severe acute pancreatitis accompanied with acute lung injury and hyperlipidemia. After hospitalization, the patient received anti-infection treatment of cefuroxime, levofloxacin and ornidazole. Considered that the primary anti-infection treatment was fail,the clinical pharmacist suggested using meropenem to cover resistant Enterobacteriaceae on the 4thday after admission.There were no improvement 5 days later.So vancomycin was empirically given for Enterococcus.And then meropenem and vancomycin were changed to levofloxacin and orndazole along with the improvement of the patient. During the treatment, clinical pharmacists helped doctors to make antimicrobial regime, optimize medication scheme of meropenem according to its PK/PD, monitor ADR of vancomycin and give medical education to patient. The patient was discharged with stable vital signs after 24 days of anti-infection treatment.
