Discussion on clinical application of amphotericin B for bladder irrigation via one patient with urinary tract infection

Abstract:One 92-year-old male patient was admitted into the hospital due to disturbance of consciousness, drowsiness for 2 days and cough for 1 day. After admission, the patient was treated with cefmetazole and moxifloxacin for anti-infection, and bladder irrigation with normal saline was given.The urine cultures revealed the Candida tropicalis for two times,while there was no urinary tract irritation symptom. Considering that the patient was old and his independent urinary function was low, the bladder irrigation with amphotericin B was provided. Combined with the characteristics of patient's medical history and related guidelines, the clinical pharmacists evaluated the rationality of amphotericin B for bladder irrigation, and provided pharmaceutical care for the patient. When adverse reactions related to amphotericin B occurred, the clinical pharmacists actively communicated with the doctor and recommended to withdraw amphotericin B. The condition of patient became stable after that.
