Pharmaceutical care of one child patient with AIH-PSC overlap syndrome and ulcerative colitis

Abstract:One 12-year-old girl was admitted to hospital due to intermittent fever, liver dysfunction for 4 months, and hematochezia for 2 months. After the completion of the relevant tests, the girl was diagnosed with AIH-PSC overlap syndrome and ulcerative colitis. Then the patient was given ursodeoxycholic acid and mesalazine. The clinical pharmacists recommended to increase the dose of ursodeoxycholic acid and add hormone. Considering that the patient was a child, physician did not adopt the recommendation of adding hormone. While, the dose of ursodeoxycholic acid was increased and hepatoprotective drugs and iron supplements were given. Clinical pharmacist provided pharmaceutical care and intervention including the effectiveness with ursodeoxycholic acid, the combination use of hepatoprotective drugs, the selection of mesalazine preparation, dosage and administration of mesalazine, the use of oral iron, and adverse reaction monitoring, etc. After the therapy, stool routine test of the patient was basically normal, liver function improved, and the patient did not have fever. The patient was discharged with stable condition.
