Medication analysis of 915 cases of adverse drug events

Abstract:Objective: To analyze the drugs used on adverse drug events (ADEs) in our hospital, search the reasons of irrational drug use and improve the safety of drug use. Methods: With the retrospective method, 488 cases of medication errors in 915 ADE cases reported by our hospital in 2016 were analyzed according to the items of unsuitable solvent selection, improper amount of solvent, inappropriate dosage of administration, unsuitable frequency of administration and dripping speed, whether or not washing the pipe before use of the following drug, with or without allergy history, etc. Results: There were 427 cases of adverse drug reaction (ADR) and 488 cases of medication errors in 915 ADE cases. There were 54 cases of unsuitable solvent selection, 44 cases of improper amount of solvent, 46 cases of inappropriate dosage of administration, 36 cases of unsuitable frequency of administration, 269 cases of unsuitable dripping speed. While, 266 cases did not wash the pipe before use of the following drug, and 50 cases had allergy history. The proportion of medication errors reduced from 55.90% in the first half of year to 50.77% in the second half of the year. Conclusion: Medication errors can be reduced by strengthening the monitoring of clinical rational drug use and standardizing the rational use of drugs.
