Pharmaceutical care on the anticoagulant therapy of a patient with pulmonary embolism after melanoma resection

Abstract:One 49-year-old male patient, who felt chest tightness and chest pain after resection of malignant melanoma of the right foot, was diagnosed as acute pulmonary embolism. The patient was given anticoagulant, anti-infective and symptomatic treatment. The clinical pharmacists carried out the whole course pharmaceutical care for the patient, including using guidelines and expert consensus to assess the disease status, develop anticoagulant regimens, adjust anticoagulant dosage according to monitoring index, monitor adverse drug reaction, conduct psychological intervention and instruct the patient to learn self-management. After the therapy, the patient reached target value of INR and was discharged with stable condition.
