Pharmaceutical care on a patient with dilated cardiomyopathy and heart failure

Abstract：One 44-year-old male patient with chest tightness and fatigue was hospitalized. The patient was given symptomatic treatment including cardiac drugs, diuretics, delaying myocardial damage, myocardial nutrition support therapy and decreasing uric acid therapy. Pharmaceutical care was given to the patient by clinical pharmacist, such as drug reforming, optimization of medication regimen for heart failure, suggestion to delay myocardial injury, rational use of levosimendan and traditional Chinese medicine, medication education, monitoring of blood pressure, heart rate, electrolyte disturbances, liver and kidney function, and urate. Therapeutic regimen was adjusted in time according to the monitoring results. And finally, the patient became better and discharged from hospital.
