Two case of acute thrombocytopenia caused by oxaliplatin for injection

Abstract:Case 1: a 60-year-old female patient was hospitalized because of the postoperation of colon cancer. During executing the chemotherapy of FOLFOX6, the patient suddenly bled from gums and nose, with subcutaneous haemorrhage after intravenous drip of oxaliplatin 1 hour. Oxaliplatin was stopped and blood routine examination was checked immediately. The results of blood routine examination showed acute thrombocytopenia. Prednisone acetate tablets (60 mg, qd), recombinant human interleukin-11 (1.5 mg, qd) and platelet transfusion (10 U·d-1) were given to the patient. The results showed that platelet count restored to normal after 13-day treatment. Case 2: a 66-year-old female postoperative colon cancer patient was admitted to hospital. During executing the chemotherapy of FOLFOX6, the patient suddenly bled from gums and nose, with subcutaneous haemorrhage after use of oxaliplatin. Blood routine examination was checked immediately. The results of blood routine examination showed acute thrombocytopenia. Prednisone acetate tablets (60 mg, qd), recombinant human Interleukin-11 (1.5 mg, qd) and platelet transfusion (10 U·d-1) were given to the patient. The results showed that platelet count restored to normal after 10-day treatment. Clinical pharmacist analyzed the possible reasons of acute thrombocytopenia from the two patients' physical condition, original disease, adverse drug reaction, and mechanism of drug action. Clinical pharmacists considered that more likely the reason was immune-mediated acute thrombocytopenia due to oxaliplatin.
