A case analysis of an off-laber use of amikacin atomization inhalation

Abstract:One 74-year-old male patient with hypertension and coronary heart disease was hospitalized because of repeated wheeze and cough for 10 years and aggravation for 4 months. Clinical diagnoses were pulmonary infection, acute exacerbation of chronic obstructive pulmonary disease and bronchial asthma. The patient was treated with imipenem and cilastatin sodium(0.5 g, q 8 h, ivgtt) and cefoperazone sulbactam (3 g, q 8 h, ivgtt) after admission. Sputum culture showed Pseudomonas aeruginosa, Acinetobacter baumannii and Klebsiella pneumonia. The drug sensitivity test of Pseudomonas aeruginosa showed that it was sensitive to intravenous piperacillin and tazobactam sodium and amikacin. Then atomization inhalation of amikacin (0.4 g, bid/qd, 29 days) was given on 8th day. The patient's pulmonary infection was improved after anti-infection treatment for three weeks. On 23rd day, systemic antibiotics transferred to intravenous piperacillin and tazobactam sodium (4.5 g, q 12 h, ivgtt). The patient was discharged after 38 days' hospitalization with improved condition.
