Pharmaceutical care on a patient with pulmonary nocardiosis after liver transplantation

Abstract:One 45-year-old male patient was admitted to hospital due to half a year after liver transplantation and had fever, cough, expectoration for 1 day. The patient was diagnosed as pulmonary nocardiosis after liver transplantation. Anti-infection treatments of compound sulfamethoxazole (SMZ-TMP) for 3 days were invalid. Then SMZ-TMP was changed to linezolid. The pharmaceutical care including evaluating the anti-infection regimen, the continuing efficacy and adverse drug effects was carried out by clinical pharmacists. Clinical pharmacists advised to use linezolid instead of SMZ-TMP according to the relevant literature and the patient's condition when SMZ-TMP didn't work. After 30 days, the patient was in stable situation and discharged from hospital.
