Design of anti-infection regime for an acute pyelonephritis patient with pan-drug resistantKlebsiella pneumoniae infection

Abstract:One 61-year-old female patient combined with a variety of basic diseases was hospitalized because of acute pyelonephritis. After hospitalization, the patient was treated with piperacillin/tazobactam (4.5 g, q 8 h). But no obvious improvement was observed. Both blood culture and urine culture results showed pan-drug resistantKlebsiella pneumoniae. Based on the results of bacterial cultures and the pharmacokinetics of antibiotic drugs, pharmacists suggested an anti-infection regime of fosfomycin sodium (2 g, ivgtt) followed by imipenem/cilastatin (0.5 g, ivgtt, 2 hour duration) for every 6 hours. Imipenem/cilastatin (0.5 g, ivgtt, 2 hour duration) were given 1 hour later after fosfomycin sodium. After ten days of treatment, the symptoms and laboratory examination indexes of the patient were back to normal.
