Pharmaceutical care on a patient with unstable angina pectoris and heart failure

Abstract:One 61-year-old male patient was hospitalized because of chest pain, chest tightness after activities, and edema of both lower extremities. The patient was given symptomatic treatment including antiplatelet aggregation, anticoagulation, antianginal therapy, diuresis, anti-hypertension, lipid-modulating, lowering uric acid. Pharmaceutical care was given to the patient by clinical pharmacist, including optimizing therapeutic regimen of heart failure, monitoring blood pressure, heart rate, electrolyte, blood lipid, the function of liver and kidney and hyperuricemia, providing medication education, adjusting therapeutic regimen according to the monitoring results. The patient discharged from hospital ifnally.
