One case of anaphylactic shock induced by iobitridol injection

Abstract:A 64-year-old female patient was admitted to hospital because of chest distress and short breath for one month and acute exacerbation of illness. She had a history of hypertension and chronic bronchitis for years, but no drug allergies. She received contrast-enhanced pulmonary CT. About 3 minutes after an IV injection of iobitridol, the patient suddenly experienced loss of consciousness and asphygmia with undetectable blood pressure. Iobitridol was stopped immediately. At the same time, she was given oxygen inhalation, cardiopulmonary resuscitation immediately, intravenous infusion of vasopressor and anti-allergy drugs, ECG monitoring, tracheal intubation for assisted breathing by ventilator as soon as possible. Her blood pressure returned to normal after 1 hour. She was given continuous intravenous pumping of dopamine, and the pumping speed was based upon the clinical need. The condition gradually stabilized after 6 hours. The patient was continuously treated with anti-infection, improvement of cardiac function and alleviation of pulmonary edema, and was transferred to a regular ward after 4 days.
