Practice and experience of clinical pharmacist adjusting antibacterial dose for critically ill patients receiving continuous renal replacement therapy

Abstract:Antibacterial doses for critically ill patients receiving continuous renal replacement therapy (CRRT) are complicated because of the heterogeneity among individual patients and the inlfuence of CRRT mechanical factors. An inappropriate dose may result in treatment failure and bacteria resistance with low blood concentration or drug toxicity with high blood concentration. In this article, cases performed pharmaceutical care by clinical pharmacists were discussed, such as carrying out special pharmaceutical care and offering individual dosing adjustment recommendations for patients receiving CRRT based on analysis of speciifc clinical conditions, PK/PD parameters and literature. These measures ensure the safety and efifcacy of drug use in anti-infective treatment.
