One case of hyponatremia with epileptic state induced by iobitridol

Abstract:One 49-year-old male patient was given iobitridol injection for abdominal CT examination due to founding thickened stomach wall for 2 days. After 3 hours, the patient developed disturbance of consciousness and twice convulsions accompanied with hyponatremia. The patient was transferred to ICU and given mannitol, methylprednisolone, midazolam, sodium valproate. The blood sodium concentration increased quickly, while the convulsions didn't occur. After 5 days, the patient was cured and discharged. Epilepsy didn't recur in the following 6 months. According to the criterion of the national adverse drug reaction (ADR) monitoring center, the relevance between iobitridol injection and this ADR for this patient was possible. During the use of nonionic contrast agents, close observation should be given to the patient in order to avoid false negative response and late response. Increasing intake of water before injecting iodinated contrast agents should be accompanied with sodium supplementation. The necessary emergency equipment and rescue medication should be ready in examination rooms in order to reply for accidental relevant ADR.
