Analysis of the real-time intervention of prescriptions based on the role of pharmacist in intervention by the contact letter

Abstract:Objective: To explore the effect on real-time intervention of prescriptions in outpatient pharmacy of our hospital. Methods:The results about the real-time intervention of prescriptions were reviewed retrospectively from January to July in 2013. A total of 1567 contact letters were collected, the basic data information was iflled in a form and the data were statistically analyzed. Results:Among 1567 contact letters, there were 10 main types of irrational prescriptions problems, such as inappropriate usage and dosage (638 cases, 40.71%), without using the standard drug name (241 cases, 15.38%), insufifcient signature (234 cases, 14.93%), unsuitable categories of prescriptions (198 cases, 12.64%), without the indication of the results of skin test (92 cases, 5.87%), inappropriate route of administration (67 cases, 4.28%), missing the diagnosis (60 cases, 3.83%), contraindication of special people (16 cases, 1.02%), repeated administration (11 cases, 0.70%), over the drug instruction (10 cases, 0.64%). The proportion of irrational prescriptions showed a clear downward trend from January (9.20%) to July (1.00%) in 2013. Conclusion: With the implement of the contact letter, pharmacists enhance the real-time intervention of prescriptions. Pharmacists can play a role in intervening the improper prescriptions by the contact letter and promote the rational drug use.
