Pharmaceutical care on a renal insufficiency patient treated with cefepime

Abstract:One 79-year-old male patient presented with 15-day cough, productive sputum and 7-day breathlessness was admitted to respiratory department. Cefepime was given empirically after admission. Considering the patient had chronic renal insufifciency for 7 years and long-term haemodialysis, clinical pharmacist paid more attention to the dosage of cefepime and its potential adverse drug reaction meanwhile suggested the dose of cefepime should be adjusted accurately according to endogenous creatinine clearance rate (Ccr) of the patient. Sufifcient communication was made between clinical pharmacist and physician when the patient developed cefepime-induced neurotoxicity. Cefepime was finally stopped and the symtom of the patient obviously improved.
