Meta-analysis on nicardipine in the treatment of hypertensive emergency

Abstract:Objective: To evaluate the therapeutic efficacy and safety of nicardipine in the treatment of hypertensive emergency by Meta-analysis and provide a reference for clinical therapy. Methods:PubMed, ISI, EMbase, CBM, CNKI, VIP and Wanfang databases were retrieved by using the key words of"nicardipine"and"hypertensive emergency"in Chinese and English, respectively. Randomized controlled trials (RCTs) on nicardipine in the treatment of hypertensive emergency were included. The quality of the included studies was evaluated. Data were analyzed by using Rev Man 5.0 software. Results: Fourteen RCTs met the inclusion criteria, including 1010 patients. Results of Meta-analysis showed that nicardipine had the better therapeutic efifcacy on reducing the SBP [MD=0.63, 95%CI (0.25, 1.01), P=0.001] and DBP [MD=0.35, 95%CI (0.15, 0.56), P=0.000 7] when compared with sodium nitroprusside. There were no statistical differences among nicardipine group, urapidil group, labetalol group, phentolamine group and nitroglycerin group. The incidence of ADR in nicardipine group was lower than that in sodium nitroprusside group [OR=0.55, 95%CI (0.38, 0.8), P=0.002], while there were no signiifcant differences among nicardipine group, labetalol group and urapidil group. Conclusion:Nicardipine was safe and effective in the treatment of hypertensive emergency.
