Current status and risk factors of unplanned readmission from perspective of outpatient follow-up in patients with heart failure with reduced ejection fraction during vulnerable phase

Abstract:Objective To observe and analyze the current status and risk factors of unplanned readmission from perspective of outpatient follow-up in patients with heart failure with reduced ejection fraction(HFrEF)during vulnerable phase.Methods A total of 162 patients in HFrEF vulnerable phase were enrolled in the study,who were admitted to Zhongnan Hospital of Wuhan University from Sept.2023 to June 2024,and their clinical data,laboratory indicators and imaging parameters were collected.The outpatient follow-up was carried out for 3 months,and then current status and risk factors of unplanned readmission were statistically analyzed in all patients.Results During the follow-up period,31 out of 162 patients in HFrEF vulnerable phase experienced unplanned readmission,with an incidence rate of 19.14%.Significant differences were observed between readmission group and non-readmission group in terms of the proportions of smoking,alcohol consumption,New York Heart Association(NYHA)classification,platelet count(PLT)<125×109/L,N-terminal pro-brain natriuretic peptide(NT-proBNP)>400 ng/L,left ventricular ejection fraction(LVEF)<40%,and malnutrition(P<0.050).The results of Logistic regression analysis showed that smoking,alcohol consumption,NT-proBNP>400 ng/L,LVEF<40%,and malnutrition were independent risk factors for unplanned readmission in HFrEF patients in vulnerable phase(P<0.050).The regression equation was:Logit(P)=5.014+1.362×(smoking)+1.228×(alcohol consumption)+1.936×(NT-proBNP>400 ng/L)+1.696×(LVEF<40%)+1.184×(m alnutrition).On this basis,the predictive value of the model was evaluated by plotting the receiver operating characteristic(ROC)curve.The results showed that when the optimal cut-off value was greater than 12.42,the area under the curve(AUC)was 0.867(95%CI:0.836~0.890),indicating that the model had a good predictive value for the occurrence of events.Conclusion Smoking,alcohol consumption,NT-proBNP>400 ng/L,LVEF<40% and malnutrition are risk factors for HFrEF patients in vulnerable phase.The regression equation and ROC curve established and plotted based on these risk factors can predict the risk of unplanned readmission with some clinical value.
