Correlation between mean arterial pressure and in-hospital mortality in patients with acute myocardial infarction

Abstract:Objective To investigate the relationship between level of mean arterial pressure(MAP)on admission and in-hospital mortality in patients with acute myocardial infarction(AMI).Methods AMI patients(n=869)were selected from Characteristic Medical Center of Chinese PLA Rocket Force from Mar.2015 to Jan.2020.The relationship between MAP admission and in-hospital mortality was statistically analyzed by using Logistic regression model,and HR and 95%CI were calculated.The relationship between MAP admission and in-hospital mortality was discussed by using restricted cubic spline(RCS)plots and inflection analysis.Results A total of 869 patients with AMI were included in this study.After adjusting for clinical features related to in-hospital all-cause mortality,MAP was an independent predictor of mortality in AMI patients.Compared with those with MAP<87.6 mmHg(1 mmHg=0.133 kPa),the patients with MAP≥98.2 mmHg had a significantly lower risk of in-hospital death,(HR=0.18,P<0.05).The results of RCS plots and inflection analyses showed an L-shaped curve of in-hospital all-cause mortality between MAP and AMI(P<0.05).In the inflection point analysis,patients with MAP<91 mmHg had a decreased risk of in-hospital mortality with increasing MAP(P<0.05).When MAP≥91 mmHg,there was no significant correlation between MAP and in-hospital mortality(P>0.05).Conclusion MAP may be an independent risk factor for in-hospital all-cause mortality,and an L-shaped relationship was found between MAP and in-hospital all-cause mortality in AMI patients.
