Influence of evolocumab on long-term prognosis in patients with non-ST-segment elevation acute coronary syndromes

Abstract:Objective To investigate the influence of evolocumab on coronary artery restenosis 1 y after PCI and major adverse cardiovascular and cerebrovascular events(MACCE)3 y after PCI in patients with non-ST-segment elevation acute coronary syndromes(NSTE-ACS).Methods NSTE-ACS patients(n=240)were randomly divided into control group and evolocumab group(each n=120).The control group was given normal medication,and evolocumab group was additionally given evolocumab treatment.After medication for 1 y,indicators including total cholesterol(TC),triglycerides(TG),high-density lipoprotein-cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),high sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6),and incidence of coronary artery restenosis were observed in 2 groups.A 3-y follow-up was conducted to compare and analyze the primary endpoints(all-cause mortality,cardiac mortality,myocardial infarction),the composite secondary endpoint of MACCE(cardiac mortality,myocardial infarction,ischemia-driven revascularization,in-stent thrombosis,stroke),as well as the incidence rates of angina pectoris,rehospitalization and major bleeding.Results At 1-y follow-up visit,the levels of indicators including TC,TG,LDL-C,hs-CRP,TNF-α and IL-6,as well as the incidence of restenosis(2.50%vs.8.33%)in evolocumab group were significantly lower than those in control group(P<0.05).During 3-year follow-up,the rates of primary endpoint events(cardiac death,myocardial infarction),secondary endpoints(total MACCE,cardiac death,myocardial infarction,in-stent thrombosis,stroke),angina pectoris recurrence and re-revascularization in evolocumab group were significantly lower than those in control group(P<0.05).Conclusion Evolocumab can effectively reduce blood fat,inhibits inflammation,reduce restenosis 1 y after PCI and MACCE incidence 3 y after PCI in NSTE-ACS patients.
