Anticoagulation effect of dabigatran etexilate in patients with non-valvular atrial fibrillation and factors associated with gastrointestinal bleeding

Abstract:Objective To explore the therapeutic effect of dabigatran etexilate on anticoagulation in patients with non-valvular atrial fibrillation(NVAF),and to analyze the risk factors associated withgastrointestinal bleeding(GIB)after dabigatran etexilate therapy in NVAF patients.Methods This retrospective study included 121 patients with NVAF treated at the First Affiliated Hospital of Hainan Medical University were selected from August 2022 to August 2024.According to different treatment methods,the enrolled patients were divided into a control group(n=57,warfarin anticoagulation therapy)and an observation group(n=64,dabigatran etexilate anticoagulation therapy).The coagulation parameters[international normalized ratio(INR),prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT)]were compared between groups before and after treatment.According to the occurrence of GIB in NVAF patients treated with dabigatran etexilate,the patients were classified into GIB group(n=22)and non-GIB group(n=42).The clinical data of patients were collected,and the risk factors of GIB were examined using multivariable logistic regression.Results Compared with the control group,the levels of coagulation parameters in the observation group were significantly higher after treatment(P<0.05).Compared with patients without GIB,patients with GIB had higher proportions of hypoproteinemia,renal insufficiency,history of cerebral infarction,cardiac insufficiency,anemia,and previous history of gastrointestinal bleeding,and higher levels of D-dimer and neutrophil percentage(P<0.05).Multivariable logistic regression indicated that history of cerebral infarction,previous gastrointestinal bleeding,and D-dimer level were factors associated with GIB in patients with NVAF receiving dabigatran etexilate therapy(P<0.05).Conclusion Dabigatran etexilate can improve the coagulation parameters in patients with NVAF.History of cerebral infarction,previous gastrointestinal bleeding,and elevated D-dimer level are factors associated with GIB in patients with NVAF treated with dabigatran etexilate.
