Abstract:Objective To explore the correlation between serum stromal interaction molecule 1(STIM1),spliced X-box binding protein(XBP1S),transcription factor 21(TCF21),and the risk of major adverse cardiovascular events(MACE)in elderly patients with hypertension and chronic heart failure(CHF).Methods A total of 208 elderly patients with hypertension and concomitant CHF admitted to the Department of Cardiology,Yuncheng Central Hospital in Shanxi Province from May 2021 to May 2024 were selected as the study group and classified by New York Heart Association(NYHA)class into Ⅰ~Ⅱ and Ⅲ~Ⅳ.Patients were divided into the MACE group and the non-MACE group based on whether MACE occurred after discharge.Another 208 healthy individuals who underwent physical examinations in our hospital during the same period were selected as the control group. The correlations between the levels of serum STIM1,XBP1S and TCF21 in the study group and the laboratory indicators were analyzed using Pearson correlation.Logistic regression was conducted to identify factors of MACE in elderly patients with hypertension and CHF.Results The levels of serum STIM1,XBP1S,TCF21,uric acid(UA),and N-terminal pro-brain natriuretic peptide(NT-proBNP)in the study group were higher than those in the control group(P<0.05).The levels of serum STIM1,XBP1S,TCF21,UA and NT-proBNP in patients with NYHA grades Ⅲ~Ⅳ were higher than those in patients with grades Ⅰ~Ⅱ(P<0.05).According to the Pearson correlation results,STIM1,XBP1S,and TCF21 were all related to UA and NT-proBNP(P<0.05).There were statistically significant differences in age and left ventricular ejection fraction(LVEF)between the non-MACE group and the MACE group(P<0.05).The levels of serum STIM1,XBP1S,TCF21,UA and NT-proBNP in the MACE group were higher than those in the non-MACE group(P<0.05).Logistic regression analysis showed that age,UA,NT-proBNP,STIM1,XBP1S,and TCF21 were all risk factors affecting the occurrence of MACE in elderly patients with hypertension and CHF(P<0.05).Conclusion Serum STIM1,XBP1S,and TCF21 are abnormally elevated in elderly patients with hypertension and CHF.All three are related to the occurrence of MACE in patients.
