Study on the relationship between preoperative aortic elasticity and the interventional treatment time window and aortic remodeling after aortic dissection

Abstract:Objective To analyze the relationship between preoperative aortic elasticity parameters and interventional treatment time window with aortic remodeling after aortic dissection,and to evaluate the predictive value of each indicator for postoperative aortic remodeling.Methods A total of 204 patients with aortic dissection who received thoracic aortic endovascular repair in Inner Mongolia Autonomous Region People's Hospital between October 2021 and October 2024 were selected as study subjects.The enrolled patients were followed up for 6 months after surgery,and were classified into positive aortic remodeling group(n=152)and negative aortic remodeling group(n=52)according to the postoperative aortic remodeling status.Aortic CT angiography was performed before surgery,and aortic elasticity parameters(aortic dilatation,aortic compliance,aortic stiffness)were obtained,and the treatment time window(<7 days,7-14 days,>14 days)was recorded.The relationship between preoperative aortic elasticity parameters and interventional treatment time window with aortic remodeling after aortic dissection was explored by Logistic regression analysis.The value of serum aortic dilatation,aortic compliance,aortic stiffness and interventional treatment time window in predicting aortic remodeling after aortic dissection was analyzed by receiver operating characteristic(ROC)curve.Results The aortic dilatation and aortic compliance in the positive aortic remodeling group were higher than those in the negative aortic remodeling group whereas aortic stiffness was lower(P<0.05).The proportion of interventional treatment time window<7 days was higher in the positive aortic remodeling group than that in the negative aortic remodeling group(P<0.05).Logistic regression analysis revealed that aortic dilatation and aortic compliance were positively associated with postoperative aortic remodeling,and aortic stiffness and interventional treatment time window were negatively correlated with postoperative aortic remodeling(OR:1.298,95%CI:1.146-1.471;OR:3.716,95%CI:1.372-10.067;OR:0.633,95%CI:0.466-0.859;OR:0.379,95%CI:0.194-0.740,P<0.05).ROC curve indicated that the area under the curve(AUC),sensitivity and specificity for the combination of aortic dilatation,aortic compliance,aortic stiffness and interventional treatment time window in predicting aortic remodeling after aortic dissection were 0.856(95%CI:0.801-0.901),71.71%and 88.46%respectively.The efficiency of combined prediction was better than that of each indicator alone(P<0.05).Conclusion Preoperative aortic elasticity and interventional treatment time window are closely related to aortic remodeling after aortic dissection.The combined evaluation of aortic dilatation,aortic compliance,aortic stiffness and interventional treatment time window can provide a certain predictive value on postoperative aortic remodeling in patients with aortic dissection,so as to guide clinical prevention and treatment and improve the prognosis of patients.
