Efficacy and safety of drug-coated balloon and drug-eluting stent to acute coronary syndrome

Abstract:Objective To compare the efficacy and safety of drug-coated balloon(DCB)and drug-eluting stent(DES)in treating acute coronary syndrome(ACS).Methods ACS patients(aged≤50,n=186),received emergency or selective percutaneous coronary intervention(PCI),were retrospectively analyzed from Nov.2019 to May 2023.All patients were divided into DCB group(n=84)and DES group(n=102).The baseline data,procedural data,and incidence of device-oriented composite endpoints(DoCE),type 2-5 bleeding events of Bleeding Academic Research Consortium(BARC)and rehospitalization due to angina were analyzed in 2 groups.The survival curves were made by using Kaplan-Meier method,and DoCE incidence was compares between 2 groups.The influence factors for outcomes were analyzed by using multivariable Cox regression analysis.Results The difference in baseline data had no statistical significance between DCB group and DES group(P>0.05).The proportion of target lesions in the left circumflex artery was higher in DCB group than that in DES group(P<0.01),while proportion of target lesions in the right coronary artery was higher in DES group than that in DCB group(P<0.05).During the procedure,there were significant differences in terms of pre-dilation balloon pressure and diameter between 2 groups(P<0.05),and balloon diameter and length were larger in DES group than those in DCB group(P<0.05).Additionally,the proportion of tirofiban administration was higher in DES group(P<0.05),and differences in other procedure data had no statistical significance between 2 groups(P>0.05).The median follow-up duration was 912(643,1,341)d in 2 groups.During follow-up period,DoCE occurred in 4 patients in each group,and rehospitalization due to angina occurred in 12 patients in DCB group and 11 in DES group.There were no type 2-5 bleeding events of BARC observed in 2 groups.The differences in DoCE incidence and rate of rehospitalization due to angina had no statistical significance between 2 groups(P>0.05).The results of multivariable Cox regression analysis showed that diabetes history,increased serum creatinine,and target lesions located in the left anterior descending artery were independent predictive factors for DoCE incidence.Conclusion Under strictly selected indications,there is no significant difference in the safety and efficacy between DCB and DES treatment in ACS patients aged≤50 years.
