Risk factors and influence on prognosis of intramyocardial hemorrhage in patients with acute myocardial infarction after percutaneous coronary intervention

Abstract:Objective To discuss the risk factors and influence on prognosis of intramyocardial hemorrhage(IMH)in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods The clinical data of AMI patients(n=120)received PCI were retrospectively analyzed,and according to IMH occurrence,the patients were divided into IMH group(n=41)and non-IMH group(n=79)in the First People's Hospital of Chuzhou and Tongling people's hospital from Jan.2022 to June 2024.The risk factors of IMH were analyzed in AMI patients after PCI,and incidence of major adverse cardiovascular events(MACE)was compared between 2 groups after PCI for 6 months.Results The results of multi-factor Logistic regression analysis showed that infarction location(OR=3.235,95%CI:1.446~7.238),patency time of coronary occlusion(OR=3.867,95%CI:1.746~8.561),number of lesion vessels(OR=2.524,95%CI:1.425~4.469),usage of GPIIb/IIIa receptor inhibitors(OR=1.842,95%CI:1.235~2.748),peak creatine kinase-MB isoenzyme(CK-MB,OR=1.137,95%CI:1.080~1.197)and left ventricular ejection fraction(LVEF,OR=0.595,95%CI:0.488~0.723)were all risk factors for complicated IMH in AMI patients after PCI(P<0.05).The results of ROC curve analysis showed that AUC of peak CK-MB combined with LVEF was 0.994 in IMH prediction,which was higher than that of peak CK-MB 0.990 and LVEF 0.868 respectively.After followed up for 6 months,MACE incidence was 19.52%in IMH group,which was significantly higher than that in non-IMH group(6.33%,P<0.05).Conclusion There are many risk factors of IMH,and have adverse influence on prognosis in AMI patients after PCI.The combination of peak CK-MB and LVEF is helpful for predicting IMH.
