Changes in intestinal flora in patients with acute myocardial infarction and their relationship with heart failure

Abstract:Objective To investigate changes in the intestinal flora of patients with acute myocardial infarction(AMI),and explore the impact of these changes on heart failure.Methods A total of 152 AMI patients treated at the First Hospital of Handan City from April 2021 to October 2023 were enrolled as the disease group,and 152 healthy volunteers during the same period served as the control group.Intestinal flora in both groups was detected by bacterial culture,and short chain fatty acids(SCFAs)in the fecal microbiota of the disease group were measured by high-performance liquid chromatography.After 30-day follow-up,AMI patients were divided into a the heart failure group and a non-heart failure group according to the occurrence of heart failure.The composition of intestinal flora,SCFA levels,,N-terminal B-type natriuretic peptide proteins(NT-proBNP)level and left ventricular ejection fraction(LVEF)were compared between the two groups.The relationship between intestinal flora and NT proBNP,LVEF in AMI patients,as well as factors influencing the development of heart failure,were analyzed.Results Compared with the control group,the disease group had significantly higher countsof Escherichia coli,Enterococcus,Clostridium,Fungi,and total aerobic bacteria(P<0.05),and significantly lower counts of Bacteroides,Bifidobacterium,Lactobacillus,and total anaerobic bacteria(P<0.05).Compared with the non-heart failure group,the heart failure group had lower counts of Lactobacillus,Bifidobacterium,Lactobacillus,total anaerobic bacteria,LVEF,acetic acid,propionic acid,butyric acid,isobutyric acid,and isovaleric acid(P<0.05),while counts of Escherichia coli,Enterococcus,total aerobic bacteria,and NT proBNP were higher(P<0.05).In AMI patients,counts of Escherichia coli,Enterococcus,and total aerobic bacteria were positively correlated with NT-proBNP(P<0.05),while counts of Bacteroides,Bifidobacterium,Lactobacillus,and total anaerobic bacteria were negatively correlated with NT-proBNP(P<0.05).Counts of Bacteroides,Bifidobacterium,Lactobacillus,and total anaerobic bacteria were positively correlated with LVEF(P<0.05),while counts of Escherichia coli,Enterococcus,and total aerobic bacteria were negatively correlated with LVEF(P<0.05).Logistic regression analysis indicated that Bacteroides,Bifidobacterium,Lactobacillus,total anaerobic bacteria,LVEF,Escherichia coli,Enterococcus,total aerobic bacteria,and NT-proBNP may be influential factors for the occurrence of heart failure in AMI patients.Conclusion AMI patients exhibit alterations in intestinal flora,which are closely associated with the development of heart failure.
