Influence of lipoprotein(a)on residual risks of coronary heart disease

Abstract:Objective To analyze retrospectively the influence of risk factors on level of lipoprotein(a)[Lp(a)]in patients with coronary heart disease(CHD).Methods CHD patients(n=10 764)admitted in the 980th Hospital of Chinese PLA Joint Logistics Support Force were selected from Sept.2015 to Dec.2021.The data of age,gender,past medical history,whether or not undergoing percutaneous coronary intervention(PCI)and results of cardiac ultrasonography were collected in CHD patients.The levels of Lp(a)and blood fat were detected,and risk factors impacting Lp(a)level was analyzed in CHD patients.Results The total level of Lp(a)was 70.05(22.60,204.48)mg/dl in all include CHD patients.Lp(a)level wa>30 mg/dl in 7470(69.40%)patients[high-Lp(a)group],and Lp(a)level was≤30 mg/dl in 3294(30.60%)patients[low-Lp(a)group].Compared with low-Lp(a)group,high-Lp(a)group had a lower proportion of males(62.02%vs.64.39%,P=0.020),a higher age[64.0(56.0,71.0)years vs.63.0(55.0,71.0)years,P=0.048],and a higher number of patients complicated by acute myocardial infarction(AMI,22.8%vs.17.4%,P<0.001)and those with low left ventricular ejection fraction(LVEF,<45%,5.74%vs.3.95%,P=0.0001).The levels of high-density lipoprotein(HDL),low-density lipoprotein(LDL)and apolipoprotein A(ApoA)were higher,and levels of triglycerides and very low-density lipoprotein(VLDL)were lower in high-Lp(a)group than those in low-Lp(a)group(P<0.05).The results of multi-factor Logistic regression analysis showed that hyperlipidemia,LVEF,LDL,HDL and ApoA were correlated to Lp(a)level,and Lp(a)level was positively correlated to lower LVEF(<45%),hyperlipidemia and HDL,and was negatively correlated to LDL and ApoA.Conclusion The increase of Lp(a)level(>30 mg/dl)is correlated to hyperlipidemia,lower LVEF(<45%)and specific lipid profile,and intervention on Lp(a)level may help to reduce residual cardiovascular risks in CHD patients.
