Analysis of the rapid lipid-lowering effect and clinical efficacy of evolocumab in patients with in-stent restenosis and different populations

Abstract:Objective To observe the rapid lipid-lowering effect of evolocumab in patients with in-stent restenosis(ISR)and its clinical effect in different populations.Methods Fifty coronary heart disease patients with ISR and uncontrolled low-density lipoprotein cholesterol(LDL-C)admitted to the 980th Hospital of the Joint Logistics Support Force,PLA,between January 2021 and December 2023 were included.All received evolocumab,and lipid parameters and LDL-C target-attainment rates were assessed at 1 and 3 months.Patients were divided into a comorbidity group(hypertension and/or diabetes)and a non-comorbidity group.Pre-treatment lipid levels and post-treatment LDL-C attainment rates were compared between groups.Results One month after treatment,LDL-C decreased significantly(P<0.01),and other blood lipid parameters did not change significantly(P>0.05).After 3 months of treatment,total cholesterol(TC),LDL-C and LP(a)were significantly decreased(P<0.01),while triglyceride(TG)and high-density lipoprotein(HDL-C)had no significant change(P>0.05).The LDL-C and LP(a)of the comorbidities group were higher than those of the non-comorbidities group before medication(P<0.01),the LDL-C compliance rate of the non-comorbidities group was higher than that of the comorbidities group at 1 month after medication(P<0.05),and there was no significant difference in the compliance rate of the two groups at 3 months(P>0.05).Conclusion Evolocumab can effectively reduce the LDL-C and LP(a)levels in ISR patients in a short time,and the LDL-C compliance rate is higher after 3 months of continuous application.For ISR patients without comorbidities,LDL-C can reach the standard faster,and with the extension of application time,the LDL-C compliance rate of ISR patients with comorbidities tends to be similarto that of ISR patients without comorbidities.
