Relationship between erythropoietin and mean platelet volume at admission and long-term prognosis in patients with coronary heart disease after percutaneous coronary intervention

Abstract:Objective To observe the relationship between erythropoietin(EPO)and mean platelet volume(MPV)at admission and long-term prognosis in patients with coronary heart disease(CHD)after percutaneous coronary intervention(PCI).Methods CHD patients(n=112)were selected and divided,according their prognosis,into good prognosis group(n=84)and poor prognosis group(n=28)at Department in Cardiology of People's Hospital of Beijing Daxing District from Jan.2021 to Dec.2022.MPV was detected by using full-automatic blood analyzer,and EPO was detected by using enzyme-linked immunosorbent assay(ELISA).The influence factors on CHD prognosis were analyzed by using multi-factor Logistics regression analysis,and predictive value of EPO and MPV to long-term prognosis was analyzed based on ROC curve analysis.Results The number of diseased coronary vessels,left ventricular ejection fraction(LVEF)<40%and Gensini integrals were significantly lower in good prognosis group than those in poor prognosis group(P<0.05).The differences in other clinical baseline data had no statistical significance between 2 groups(P>0.05).The major adverse cardiovascular event(MACE)wwas post-PCI angina pectoris(35.71%)in poor prognosis group.EPO and MPV at admission were significantly lower in good prognosis group than those in poor prognosis group(P<0.05).The number of diseased coronary vessels,LVEF<40%,EPO>8.91 mU/L and MPV>9.23 fl were independent risk factors for MACE occurrence in CHD patients after PCI(P<0.05).The probability by risk prediction model was as follow:P=1/[1+e(-4.296+1.257×(number of diseased coronary vessels)+1.288×(LVEF)+1.207×(EPO)+1.239×(MPV)].The AUC of EPO combined with MPV was 0.918 in predicting MACE,sensitivity was 92.86%and specificity was 76.19%.AUC and sensitivity were the highest.Conclusion EPO and MPV at admission are correlated to long-term prognosis in CHD patients after PCI.
