Effect of eloucumab on plasma resistin level in patients with acute coronary syndrome

Abstract:Objective To investigate the effect of eloucumab on plasma resistin level in patients with acute coronary syndrome(ACS)after percutaneous coronary intervention(PCI).Methods A prospective randomized controlled study was conducted,enrolling 101 ACS patients undergoing PCI.They were randomly assigned to an observation group(n=50)and a control group(n=51).Both groups received 4-week treatment:The control group received monotherapy with Atorvastatin Calcium Tablets(20 mg/d),while the observation group received subcutaneous injection of evolocumab(140 mg)within 12 hours pre-or post-operatively in addition to atorvastatin,followed by 140 mg administered every two weeks,totaling a 4-week treatment course.Serum levels of Proprotein Convertase Subtilisin/Kexin Type 9(PCSK9),resistin,blood lipid profiles,and inflammatory markers were measured at baseline and 4 weeks of treatment in both groups,with adverse events recorded throughout the study.Results During follow-up,10 patients in the observation group and 11 in the control group were lost to follow-up,resulting in 40 patients being analyzed per group.No significant differences were observed in baseline characteristics between the two groups before treatment(P>0.05).Compared to baseline,the control group showed no significant changes in PCSK9 or resistin levels after 4 weeks of treatment(P>0.05).In contrast,the observation group exhibited significant reductions in both plasma PCSK9 and resistin levels(P<0.05),with statistically significant intergroup differences versus the control group(P<0.05).After 4-week treatment,both groups demonstrated significant decreases from baseline in LDL-C,total cholesterol(TC),and triglycerides(TG),(P<0.05).Notably,the reductions in LDL-C,TC,and TG were significantly greater in the observation group than in the control group(P<0.001).However,no significant difference was observed in the reduction of HDL-C either within or between the two groups(P>0.05).IL-6 and TNF-α levels decreased significantly from baseline in both groups post-treatment(P<0.05),whereas high-sensitivity C-reactive protein(hs-CRP)levels showed no statistical significance(P>0.05).During the treatment period,11 major adverse cardiovascular events(MACE)occurred,with incidence rates of 5.0%(observation)versus 22.5%(control),revealing a significant intergroup difference(P<0.05).Correlation analysis indicated positive associations between plasma resistin levels and PCSK9,TG,LDL-C,IL-6,and TNF-α(r>0,P<0.05).Stepwise regression analysis identified TG,IL-6,and TNF-α as independent predictors of resistin levels.Conclusion Early Evolocumab administration reduces plasma resistin levels in patients with ACS,alleviates chronic inflammatory status in atherosclerosis and related cardiovascular diseases,and decreases the incidence of MACE.
