Observation on the effect of different pretreatment balloons in preventing restenosis after drug-coated balloon interventional therapy for patients with coronary heart disease

Abstract:Objective To observe the effects of different pre-treatment balloons in preventing restenosis after drug-coated balloon interventional therapy in patients with coronary heart disease.Methods A total of 147 patients with coronary heart disease admitted to Cangzhou People's Hospital from January 2022 to June 2023 were selected.All patients underwent drug-coated balloon(DCB)percutaneous coronary intervention.Based on different balloon pre-dilatation strategies prior to DCB intervention,the patients were divided into three groups,with 49 cases in each group.Group A received semi-compliant balloon pre-dilatation before DCB intervention,Group B received scoring balloon pre-dilatation,and Group C received cutting balloon pre-dilatation.Intravascular ultrasound(IVUS)was used to evaluate the clinical efficacy of each pre-dilatation strategy in DCB intervention.The rates of post-operative restenosis,recurrence of angina pectoris,incidence of major adverse cardiovascular events(MACE),and the occurrence of surgical complications were statistically analyzed for each group.Results The total effective rate in Group C was 91.84%,which was higher than that in Group A(73.47%)and Group B(89.80%).After treatment,the minimum stent cross-sectional area,the minimum lesion vessel area,and the minimum vessel diameter in all three groups increased compared to those before treatment(P<0.05).However,the minimum stent cross-sectional area in Group A was smaller than that in the other groups,and the minimum lesion vessel area in Group B was smaller than that in the other groups(P<0.05).Moreover,there were no statistically significant differences in the reference vessel diameter(LDr)and the minimum vessel diameter among the three groups(P>0.05).Although the degree of post-operative stenosis in Group A was lower than that in the other groups,it had the highest rates of restenosis and complications,at 26.53%and 28.57%,respectively(P<0.05).There were no significant differences in the rates of restenosis and complications between Groups B and C(P>0.05).Conclusions Different pre-dilatation methods for balloons were employed before drug-coated balloon(DCB)intervention for coronary heart disease.Post-operatively,scoring balloon and cutting balloon pre-dilatation demonstrated better vascular dilation effects compared to semi-compliant balloon pre-dilatation,with lower rates of post-operative restenosis and complications in patients.However,the lumen area obtained after scoring balloon pre-dilatation was slightly inferior.Therefore,cutting balloon pre-dilatation exhibits higher efficacy and safety in this context.
