Correlation between neutrophil percentage to albumin ratio and prognosis in elderly patients with heart failure

Abstract:Objective To relationship between neutrophil percentage to albumin ratio(NPAR)and poor prognosis in elderly patients with heart failure(HF).Methods The elderly HF patients(n=1683)were selected from HF database,and divided,according to NPAR tertiles,into T1 group(NRAR<18.95),T2 group(18.95≤NPAR<22.45)and T3 group(NPAR≥22.45).The data were analyzed by using SPSS 27.0 and R4.2.2 statistical software,and relationship between NPAR and all-cause mortality was analyzed in elderly HF patients after 28 d,3 months and 6 months.The dose-response relationship between NPAR and the prognosis was reviewed by using restricted cubic spline(RCS)plots.A subgroup analysis was conducted to determine the consistency of correlation between NPAR and 28-day mortality across different populations.The predictive effects of NPAR,neutrophil percentage and albumin(ALB)were reviewed by using ROC curves.Results The results of Multi-factor Logistic regression analysis revealed that for each unit increased in NPAR,the risk of all-cause mortality increased,respectively,by 8.8%,8.2%,and 6.4%at 28 d(OR=1.088,95%CI:1.022~1.157),3 months(OR=1.082,95%CI:1.020~1.148)and 6 months OR=1.064,95%CI:1.009~1.123,P<0.05).Taken T1 group as reference group,T3 group exhibited the highest mortality risk at 28 d(OR=5.692,95%CI:1.537~21.080),3 months(OR=3.621,95%CI:1.253~10.460),and 6 months(OR=3.237,95%CI:1.304~8.041,P<0.05).The results of RCS analysis showed that NPAR positively correlated to all-cause mortality at 28 d,3 months and 6 months(P-nonlinear>0.05).There was no interactive effect among subgroup factors,NPAR and 28-d all-cause mortality(P for interaction>0.05).The results of ROC curve analysis showed that,in predicting 28-d all-cause mortality,AUC of NPAR was 0.719(95%CI:0.634~0.805),AUC of neutrophil percentage was 0.687(95%CI:0.598~0.776),and AUC of ALB was 0.672(95%CI:0.586~0.759,P<0.05).Conclusion High NPAR is an independent risk factor for all-cause mortality,and NPAR has a higher value in predicting all-cause mortality compared with neutrophil percentage and ALB in elderly HF patients.
