Correlation between systemic immune inflammation index and poor prognosis in patients with acute heart failure

Abstract:Objective To study the correlation between systemic immune inflammation index(SII)and poor prognosis in patients with acute heart failure(AHF).Methods AHF patients were selected into AHF group(n=272),and meanwhile healthy adults with physical examination were selected into control group(n=272)from Department of Cardiology in the Fifth Affiliated Hospital of Zhengzhou University from Jan.2020 to Dec.2021.The clinical data,including blood cell counts,N-terminal pro-brain natriuretic peptide(NT-proBNP),blood sugar,blood fat,left ventricular ejection fraction(LVEF),NYHA classification of heart function and clinical medication,were collected in AHF group.According to SII median,AHF group was divided into low-SII group and high-SII group(n=136).The all-cause mortality and composite endpoint events were followed up within 12 months after AHF group discharged.Results SII value increased significantly in AHF group compared with control group(P<0.05).The platelet count(PLT),neutrophil count(NEUT),and levels of blood sugar and NT-proBNP were higher,and lymphocyte count(LYM)and LVEF were lower in high-SII group than those in low-SII group(P<0.05).The all-cause mortality and incidence of composite endpoint events increased significantly in high-SII group than those in low-SII group(P<0.05).The results of ROC curve analysis showed that,in SII predicting composite endpoint events,AUC was 0.861(95%CI:0.819~0.903,P<0.05),sensitivity was 88.1%and specificity was 76.4%.The results of multi-factor Cox proportional hazard model showed that SII was an influence factor for composite endpoint events in AHF patients(P<0.05).Conclusion SII value increases significantly and has higher predictive value to poor prognosis in AHF patients.
