Influence of delayed revascularization strategy based on fractional flow reserve in patients with coronary heart disease complicated by type 2 diabetes mellitus

Abstract:Objective To study the curative effect of delayed revascularization strategy based on coronary angiography(CAG)and fractional flow reserve(FFR)in patients with stable angina pectoris(SAP)complicated by type 2 diabetes mellitus(T2DM),and observe its influence on prognosis.Methods The patients with SAP complicated by T2DM(n=380)with FFR value from 0.8 to 0.85 were selected at Qingpu Branch of Zhongshan Hospital affiliated to Fudan University from Apr.2022 to July 2023.All patients were divided,according to random digital table,into observation group and control group(each n=190).The control group was only given relevant drug treatment recommended by current clinical guidelines,and observation group was additionally given more active percutaneous coronary intervention(PCI).The incidence of major adverse cardiovascular events(MACE)was followed and observed,including all-cause death,cardiac death,non-fatal myocardial infarction and unplanned target vessel re-revascularization(TVRR),and angina pectoris and exercise tolerance were reviewed in 2 groups within 1 y after discharge.Results The differences in baseline data(P>0.05)and 1-y MACE incidence(P>0.05)had no statistical significance between 2 groups.The scores in 5 dimensions of Seattle Angina Questionnaire(SAQ),including physical limitation(PL),angina stability(AS),anginal frequency(AF),treatment satisfaction(TS)and disease perception(DP),were higher in observation group than those in control group(all P<0.05).After treatment,the levels of metabolic equivalent(MET)and peak oxygen consumption(VO2max,P<0.001)were higher,and carbon dioxide ventilation equivalent slope(VE/VCO2 slope)was lower in observation group than those in control group(P<0.001).Conclusion The delayed revascularization strategy based on FFR has no effect on relieving angina symptoms and increasing exercise tolerance in patients with SAP complicated by T2DM treated with conservative drug therapy.
