Electrocardiography features of acute total occlusion of coronary artery in patients with dialysis for end-stage renal disease complicated by coronary artery disease

Abstract:Objective To analyze the electrocardiography(ECG)features of acute total occlusion of coronary artery(ATO)in patients with dialysis for end-stage renal disease(ESRD)complicated by coronary artery disease(CAD),aiming at early identification,recovering revascularization and improving prognosis.Methods The medical records collected from patients with dialysis for ESRD complicated by CAD in 6 participating centers through utilizing Coronary Revascularization in Patients with End-stage Renal Disease on Dialysis in China:a Nationwide and Multicenter Cohort Study(CRUISE-R)-China's largest cohort.The dialysis patients with ATO of CAD were selected into dialysis group(n=72)from the following hospitals met the inclusion criteria:China-Japan Friendship Hospital,Beijing Chaoyang Hospital(affiliated to Capital Medical University),Xinjiang Autonomous Region People's Hospital,Ningbo First Hospital,Shaoxing People's Hospital and Chengdu Third People's Hospital.The non-dialysis patients with ATO of CAD were consecutively enrolled and matched into non-dialysis group(n=260)from Cardiology Department of China-Japan Friendship Hospital during the same period.The two groups had comparable gender and age distributions(aged from 18 to 80).Based on ECG features in 2 groups,ECG features were analyzed in dialysis patients with ATO of CAD.Results The most frequent ECG features were precordial unstable T waves and ST-segment depression in dialysis group,while ST-segment elevation was the predominant feature in non-dialysis group.Both groups showed consistent incidences of De Winter ST/T complex waves and T-wave inversion.Conclusion In dialysis patients with ATO of CAD,ECG demonstrates that ST-T changes mainly characterized by ST-segment depression and a high prevalence of precordial unstable T waves.ECG can provide early warning for patients with atypical clinical manifestations.Enhanced attention is needed to reduce delays in reperfusion therapy and incidence and mortality of cardiovascular events.
