Predictive value of miRNA-1 and miRNA-208a detection for acute myocardial infarction in patients with myocardial injury

Abstract:Objective To analyze the expression levels of plasma microRNA-1(miRNA-1)and microRNA-208a(miRNA-208a)in patients with myocardial injury and evaluate their diagnostic value in predicting acute myocardial infarction(AMI).Methods A case-control study was conducted,enrolling 90 patients with elevated cardiac troponin I(cTnI)admitted to Jinshan Hospital affiliated with Fudan University from June 2022 to May 2023.These included 48 AMI patients(AMI group),42 myocardial injury patients(myocardial injury group),and 45 hospitalized patients with normal cTnI levels as controls(control group).Plasma levels of miRNA-1 and miRNA-208a were measured using quantitative polymerase chain reaction(qPCR).Logistic regression was used to identify risk factors for AMI,and receiver operating characteristic(ROC)curves were analyzed to assess the predictive value of miRNA-1 and miRNA-208a in myocardial injury patients for AMI.Results Compared with the control group,cTnI,N-terminal pro-brain natriuretic peptide(NT-proBNP),miRNA-1,and miRNA-208a levels were significantly elevated in both the AMI and myocardial injury groups(P<0.05).The AMI group showed significantly higher total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),cTnI,miRNA-1,and miRNA-208a levels,but lower NT-proBNP levels compared to the myocardial injury group(P<0.05).Logistic regression revealed that miRNA-1 and miRNA-208a were positively correlated with AMI(OR=5.222,95%CI:1.917~8.526,P<0.01;OR=1.908,95%CI:0.048~3.768,P<0.05,respectively).ROC analysis demonstrated that the area under the curve(AUC)for plasma miRNA-1 and miRNA-208a in predicting AMI among myocardial injury patients was 0.910(95%CI:0.850~0.970)and 0.843(95%CI:0.763~0.922),respectively.Conclusion Elevated plasma miRNA-1 and miRNA-208a levels in myocardial injury patients are risk factors for AMI and exhibit high diagnostic value for predicting AMI in this population.
