Relationship among ratio of high-density lipoprotein-cholesterol to low-density lipoprotein-cholesterol,ratio of blood urea nitrogen to creatinine and prognosis in patients with acute exacerbation of chronic heart failure

Abstract:Objective To explore the relationship among ratio of high-density lipoprotein cholesterol to low-density lipoprotein cholesterol(HDL-C/LDL-C),ratio of blood urea nitrogen to creatinine(BUN/Cr)and prognosis in patients with acute exacerbation of chronic heart failure(CHF).Methods A total of 340 patients with acute exacerbation of CHF were selected in Chengdu West District Hospital from July 2020 to July 2023.They were followed up for 6 months and divided into a good prognosis group and a poor prognosis group according to whether they had adverse cardiovascular events.The clinical data,levels of HDL-C/LDL-C and BUN/Cr were compared between 2 groups.The diagnostic value of HDL-C/LDL-C and BUN/Cr to prognosis was analyzed through plotting receiver operating characteristic(ROC)curves,and risk factors for poor prognosis were analyzed by using multi-factor Logistic regression.Results Of the 340 patients,25 were lost to follow-up,and remaining 315 completed the follow-up.Among them,83 cases(24.41%)had a poor prognosis,and 232 cases(75.59%)had a good prognosis.There were no statistically significant differences in terms of age,gender,disease duration,history of hypertension,history of diabetes,cardiomyopathy,smoking history,drinking history,or proportion of diastolic blood pressure(DBP)between 2 groups(P>0.05).The proportion of NYHA class Ⅳ,left ventricular ejection fraction(LVEF)and systolic blood pressure(SBP)were all lower in good prognosis group than those in poor prognosis group(P<0.05).HDL-C/LDL-C was higher and BUN/Cr was lower in good prognosis group than those in poor prognosis group(P<0.05).The AUC values of HDL-C/LDL-C and BUN/Cr were 0.595 and 0.909,respectively,both lower than the AUC value of 0.915 for their combined diagnosis(P<0.05).The results of Logistic regression analysis showed that a high proportion of NYHA class Ⅳ,a low HDL-C/LDL-C and a high BUN/Cr were risk factors for poor prognosis(P<0.05).Conclusion HDL-C/LDL-C and BUN/Cr are risk factors for poor prognosis in CHFpatients.They have high efficacy in predicting poor clinical prognosis and thus possess good clinical application value.
