Application of"face-to-face"technique in root management surgery for acute type A aortic dissection

Abstract:Objective This study aims to compare the effects of conventional root treatment and the"face-to-face"technique in the surgical management of acute type A aortic dissection(ATAAD).Methods A retrospective analysis was conducted on the clinical data of 120 patients with ATAAD admitted to the Department of Cardiac Surgery at Zhengzhou Seventh People's Hospital from January 2021 to June 2024.Based on the different methods of aortic root treatment,the patients were divided into a conventional treatment group(67 cases)and a face-to-face group(53 cases).The basic characteristics,perioperative indicators,incidence of aortic valve regurgitation,left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVDd),and postoperative neurological complications were compared between the two groups.Results The perioperative indicators-including occlusion time,hemostasis time,surgery duration,ICU stay length,hospitalization period,and postoperative drainage volume at 24 and 48 hours-were significantly lower in the face-to-face group compared to those in the conventional group(P<0.05).Additionally,the proportion of patients experiencing aortic valve regurgitation was significantly reduced in the face-to-face group relative to that in the conventional group(P<0.05).No significant differences were observed between both groups regarding LVEF,LVDd,or postoperative neurological complications(P>0.05).Conclusion The"face-to-face"technique for root treatment in ATAAD patients demonstrates safety and efficacy;thus it merits further clinical promotion and application.
