Clinical characteristics in patients with antiphospholipid syndrome complicated by acute coronary syndrome

Abstract:Objective To analyze the clinical characteristics and risk factors in patients with antiphospholipid syndrome(APS)complicated by acute coronary syndrome(ACS).Methods The clinical data of patients with APS complicated by ACS was retrospectively analyzed at the First Affiliated Hospital of Tsinghua University from Jan 2019 to Dec.2022.The general conditions,clinical manifestations,auxiliary examinations,treatment and outcomes were summed up and analyzed,and risk factors for ACS occurrence were analyzed in APS patients.Results There were totally 23 patients with APS included in this study,and there were 8 patients with APS complicated by ACS.The percentage of male cases,age,smoking proportion,hypertension,hyperlipidemia,increased homocysteine(Hcy),positive of lupus anticoagulant(LA)and integrals of adjusted Global Antiphospholipid Syndrome Score(aGAPSS)were higher in ACS group(n=8)than those in non-ACS group(n=15).Except of increases of coronary plaque or intracardiac thrombus and intracardiac thrombus in ACS group(P<0.001),there were no statistical differences in important arteriovenous thrombosis events between 2 groups,including stroke,pulmonary embolism,deep vein thrombosis(DVT),thrombocytopenia and hemolytic anemia.Male,smoking,hypertension,LA positive and high-risk aGAPSS integrals(≥6)were risk factors for ACS occurrence in APS patients.Conclusion Male,smoking,hypertension,LA positive and high-risk aGAPSS integrals are risk factors for ACS occurrence in APS patients.
