Correlation between serum β2-microglobulin and clinical prognosis in patients with hypertension complicated by stages 3-5 chronic kidney disease

Abstract:Objective To investigate the relationship between serum β2-microglobulin(β2-MG)and long-term prognosis in patients with hypertension complicated by stages 3-5 chronic kidney disease(CKD).Methods There were totally 615 patients selected into this retrospective cohort study.According to level of serum β2-MG,the patients were divided into group 1(β2-MG<38.8 mg/L,n=204),group 2(38.8 mg/L≤β2-MG≤47.8 mg/L,n=202)and group 3(β2-MG>47.8 mg/L,n=209).The patients in 3 groups were followed up every 3 months through outpatient visits or telephone interviews for 4 y.The outcome events were recorded including all-cause death and cardiovascular death.The survival rates were compared by using Kaplan-Meier survival curve.The relationship between β2-MG level and prognosis was assessed by using Cox proportional hazards regression models and curve of restricted cubic spline(RCS)in patients with hypertension complicated by stages 3~5 CKD.Results The results of Kaplan-Meier curve analysis showed that as β2-MG level increased,the all-cause mortality(log-rank P=0.029)and cardiovascular mortality(log-rank P=0.033)increased significantly in group 3 compared those in group 1.The results of multi-facto Cox regression analysis showed that risk of all-cause death increased significantly in group 2[HR=1.53,95%CI(1.02~2.30)]and group 3[HR=1.62,95%CI(1.08~2.41)]compared those in group 1,and risk of cardiovascular death increased significantly in group 3[HR=1.73,95%CI(1.08~2.77)].The results of RCS curve analysis showed that β2-MG had a monotonic positive relationship with all-cause death in patients with hypertension complicated by stages 3~5 CKD(nonlinear P=0.942).The results of subgroup analysis showed that the correlation between β2-MG and risk of all-cause death was more significant in elderly patients(P=0.023),and correlation between β2-MG and risk of cardiovascular death was more significant in elderly patients with coronary heart disease(CHD,age P=0.002,CHD P=0.048).Conclusion The level of β2-MG is positively correlated to long-term risks of all-cause death and cardiovascular death in patients with hypertension complicated by stages 3-5 CKD..0
